The Geriatrics Education and Care Revolution: Diverse Implementation of Age-Friendly Health Systems
The Health Resources and Services Administration (HRSA) developed the Geriatrics Workforce Enhancement Program (GWEP) to maximize training for clinicians, health professional students, faculty, community-based organizations, families, caregivers, and older adults. The program’s purpose is to improve health outcomes for older adults by developing a healthcare workforce that maximizes patient and family engagement, integrating geriatrics and primary care using the Age-Friendly Health Systems (AFHS) framework, known as the 4Ms: What Matters, Medication, Mentation, and Mobility. This paper highlights an innovative and effective 4Ms framework for primary care implementation in three of the current 48 GWEPs.
