
Implementation of Anxiety Screening in Lung Transplant Candidates
Background: Adult patients with end-stage lung disease who are referred for a transplant undergo comprehensive evaluations. The International Society for Heart and Lung Transplantation’s recommends the evaluation of mental health conditions as they can lead to suboptimal post-transplant outcomes (Dew et al., 2018).
Local Problem: A screening tool is not utilized to assess anxiety presence during lung transplant evaluation at Johns Hopkins Hospital. Methods: Lewin’s Theory of Planned Change guided this project. This Quality Improvement project aimed to assess readiness to incorporate anxiety screening during lung transplant evaluation and the ability to complete the screening.
Intervention: Lung transplant candidates completed the Generalized Anxiety Disorder-7 (GAD-7) at their first clinic visit. The lung transplant team completed pre-and post-implementation Likert scale surveys of 0-10 to assess changing practice of using the GAD-7 screening tool.
Results: 12 out of 17 screening tools were completed for 71% completion rate. The pre-and post-implementation survey compared by chi square was p=0.74 for importance, p=0.39 for readiness, and p=0.054 for confidence with team rating all aspects five and above. No difference was statistically significant from pre- to post-implementation since all p values were 0.5 or greater. Most open-ended responses on the survey were in support of using the GAD-7.
Conclusion: Achievement of 71% completion rate shows feasibility is high and the team remained in favor of using the GAD-7 since all aspects were rated at a five or greater with no significant change from pre-to post and positive remarks. Due to the limited time on the first clinic visit, the team felt administration was better suited at a different clinic visit.
