Automated Message Reminder for Screening Colonoscopies

Background: Inadequate bowel preparation for a screening colonoscopy decreases the likelihood of detecting a cancerous lesion and increases the risk of adverse events. Paying a higher deductible or two co-payments can become a financial burden for patients if they need to reschedule the procedure due to inadequate bowel preparation. Rescheduling patients due to inadequate bowel preparation decreases availability for other patients to schedule an endoscopy procedure. Local Problem: Approximately 20% of patients arrive at the project site with inadequate bowel preparation which requires rescheduling of the procedure. Patients may wait up to six months for a rescheduled appointment. Methods: Quantitative statistical data was collected including Boston Bowel Preparation Score (BBPS) through chart reviews post-procedure, patient satisfaction surveys post-procedure, nurse satisfaction surveys post-implementation, and appointment reschedule rates.
Interventions: Automated reminders were sent to patients scheduled for screening colonoscopies prescribed Nulytely bowel preparation for increased education and to assist with adequate bowel preparation. Results: The rate of rescheduled procedures was 11.1% which was a decrease from the 20% average. Patients and nurses enjoyed this service and would like to see it in the future based on post-procedure and post-implementation surveys. Conclusions: Automated reminders regarding bowel preparation increased BBPS scores; patients and nurses found additional education helpful and would like to see this be utilized in the future with all patients for screening colonoscopies.
