Healthcare Transformation: Meeting Health Care Needs with the Patient Centered Medical Home

The United States spends more per capita than any other nation on healthcare, but ranks last among developed countries in health indices. Despite this investment in healthcare, the problems of poor access to care, escalating costs, and erosion of primary care persists. Healthcare is delivered in an episodic, uncoordinated manner adding to the cost while fewer then 50% of patients with chronic disease receive the care they need.
