Reducing Readmission Rates

The overall purpose of this research study is to reduce hospital readmission rates and increase patient satisfaction through education throughout the hospital stay. In the hospital setting, how does patient education upon admission and throughout the hospital stay compared to at discharge affect readmissions, patient outcomes, and patient satisfaction. It’s important for patients to understand their care. The quality of the nursing care helps to educate patients which decreases readmissions. Coordinating care with the interdisciplinary team, providing quality education, and learning what your patients already know is a good start to improving their hospital visit and decreasing their chances of being readmitted. This research study will provide evidenced based practices throughout the paper. Hear the audio explanation of this project at https://bit.ly/2Ve9MCm
